You have chronic idiopathic urticaria I think. (Hives).
See the link for more info.
Sometimes if I take 3 prescription drugs too close together I get it and it feels like my hands and other places where it comes up are actually on fire. Moving the drugs so they're taken several hours apart got rid of it. Also some cheapo red wines will start it off in me, fortunately I now know which ones it was.
I think you need to be really persistent with your doctor because this condition is spoiling your life. Also, cut the high strength multi-vits and any fish oils for a while and find something that's high in vitamin E (good for skin tissues) like cold pressed wheatgerm oil capsules or a little bottle of liquid which is not so expensive.
Good luck!

You could have a rare condition in your nervous system that causes nerve endings to constantly tell your brain that there's something irritating the skin. This seems most likelyy, especially as you have no visible symptoms. I don't know the same of the condition (sorry!) but i have heard about it before, but, as i say, it's extremely rare, so it could be, it could not be, ask your doctor if it could be neurological or nerve related, if they say it could be, then get areferrall to a neurologist.
Hope this helps :)
