1This is a very good check list, I would only add a space for "special care instructions" in case the parents want to add personal info of their own.
I would not delete anything, contrary to some other answerers who thought you should delete the gun thing. The ppl you are babysitting for should know that you don’t feel safe with guns lying around, and if they are offended by that, then you definetely don’t want to babysit for them anyway, in moho.

6thats such a good idea! I hate it when I’m baby sitting and I don’t know their names or allergies I’m definitely stealing this form!!
Also you should add one about house rules cuz the kids I babysit love to take advantage of the fact that I don’t know their rules!

7maybe just take out the children’s names out because they may think if you cant remember the kids name then you cant watch them lol

9oh wow. yes i think this is a good thing to give to the parents they may find it a bit weird but they will understand its good to know where everything is and what you did.
I dont think you could possibly add anything else. or wait ya!! if you have permission to go on the computer or phone while the child(ren) is sleeping! but other wise very good

10Wow, I wish you lived near me…….you sound like a great responsible baby sitter. Your list is just fine. The parents can leave out anything that doesn’t pertain to them. Most parents already would have this information available to you. But at least your thinking.

11take out the "things we should hide?" those things are obviously dangerous for kids so u shouldnt have to ask if u shud hid em. also take out that part about alarm codes, people dont wanna be spreading that type of info around

12Don’t take the gun thing off.
I think it’s thorough and shows you are organized. I like that you will be filling out a report for them as well, and make sure they know that will be coming as it’s a perk for parents.

13pretty good job.
maybe take out the things that would like to be locked up or hidden because they should already be locked up or hidden for obvious reasons so there is no need for that.
and maybe next to the alarm/security code put in ( ) optional because some people are very private about that kinda stuff and dont want to give it out.
ask about specific rules they must follow stuff like bedtimes and they can do this but not this type of thing. and then if they were to break a rule what type of punishment (if any) that they would like you to give them. such as time out.
